Pick Form 2 when you need tailored limits (business assets, specific gift restrictions, complex donee panels). Everyone else should default to Form 1 unless counsel says otherwise.

  • LPA Certificate Issuer professional fee (doctor / lawyer / psychiatrist on OPG’s list) — separate, not set by OPG

  • Later medical reports when a donee needs to prove lack of capacity to a bank

Decision rule

If you hold Singapore bank accounts, SRS, or property interests — EP or PR — make an LPA while capacity is clear. Default to Form 1, budget S$30 (PR) or S$160 (foreigner) for OPG plus Certificate Issuer fees, and store donee instructions next to your will and CPF nomination map. Deputyship after a crisis is the slow, expensive alternative this fee table is trying to help you avoid.
