EP medical examination for IPA issuance

If your IPA requires a medical after arrival, use the form from your IPA or HR, not a Work Permit PDF. HR can seek an STVP extension if labs run late.

Short answer: Read your own IPA. It says whether you need a medical examination after you arrive, and some EP candidates only have to submit a medical declaration rather than sit an exam. If yours asks for a full exam, get it done in Singapore and have the results ready for HR to upload when they issue the pass in EP eService. Don't assume the Work Permit medical_form.pdf from a web search applies to you, because it doesn't. And if the labs are slow, HR has to request the STVP extension 7 to 14 days before expiry with a doctor's memo, not after you've already overstayed.

r/askSingapore keeps folding three different things into one: IPA-required EP medicals, Work Permit medical PDFs, and employer pre-employment screens done for insurance or fitness. Only the first of those feeds into issuance, and only your IPA tells you whether it applies to you.

When the IPA says medical

MOM's apply page puts it on the IPA: your letter states whether you need a medical examination after arrival in Singapore. The IPA is also a single-entry visa that gives you 6 months to enter and get the pass issued, so the medical has to fit inside that window. When the employer goes to issue the pass, the uploads can include a completed medical examination form or a medical declaration form, along with the declaration form attached to your IPA, signed by authorised HR or an employee at managerial level or above.

Declaration or full exam

Which one you get is a risk call made for you. MOH told Parliament on 2 March 2026 that long-term pass applicants have to be screened for active TB with a chest X-ray on arrival in Singapore, except selected EP applicants. Because TB prevalence among EP holders is low, those applicants submit a medical declaration that they're free of TB instead.

When MOH extended the medical to Employment Passes in 2000, it described the exam as a general physical examination, a chest X-ray and an HIV test, done by a registered GP in Singapore, with issuance subject to the outcome of the report. If your IPA asks for a full exam today, the form that comes with it lists the exact tests, so go by that rather than by anyone's memory of theirs.

The clinic run, from the candidate's side

  1. Confirm the exact form first, from the IPA attachment or HR's pack, and from nowhere else.
  2. Book a GP clinic that does work pass medicals, and ask whether the chest X-ray and blood tests happen on site or get sent out. Same-day X-ray saves a second trip, so call before you go rather than turning up hopefully.
  3. Bring your passport, your IPA, and a local mobile number. Results may come back on a separate collection date, and someone will need to reach you.
  4. Ask the clinic for that collection date in writing before you leave. If it lands near the end of your visit pass, it's the line HR needs on the doctor's memo.
  5. Ask who pays before you tap your card. Plenty of employers reimburse and some pay the panel clinic directly, but you want that answer in writing beforehand, not in an expenses argument later.
  6. Hand the results or sealed forms to HR the same day they're ready, so issuance can move while your visit pass is still valid.

Don't try to get ahead by doing the X-ray at home before you fly. MOM's page puts the medical after arrival, and MOH describes the X-ray as screening on arrival into Singapore. An overseas check-up, however recent, doesn't stand in for the form your IPA names.

Personal medicines you're bringing through Customs are a different guide altogether: HSA personal medication.

Privacy and pre-existing conditions

Forum heat on this topic tends to mix the TB chest X-ray up with a much broader worry, which is whether MOM or HR ends up seeing your entire medical history. Worth pulling those apart.

The screens are aimed at public health and fitness to work: TB, HIV, and the clinical checks listed on the form. They aren't an insurance underwriting exercise. If you have a known condition the form asks about, answer it, and bring an English specialist letter on letterhead so the doctor has context, rather than going quiet on the questions. If you're worried about what gets reported, ask the clinic what goes back on the form before the exam starts. Employer group insurance underwriting is a separate exercise from the MOM issuance medical, which we cover in the health insurance audit.

One condition is restricted in law, not just screened for. MOH confirmed to Parliament in February 2025 that immigration rules let foreigners living with HIV enter Singapore as short-term visitors but restrict their long-term stay. If that applies to you, have the conversation with HR before you hand in notice at home, not at the clinic.

When the labs run past your visit pass

Three clocks run at once in your first weeks here, and none of them knows about the others.

ClockWho owns itWhat goes wrong
The IPA medicalYou and the clinicWrong form (the Work Permit PDF), an incomplete panel, a results pickup that slips
Your visit pass end dateYou, and whoever at HR files the requestThe request misses MOM's window and you start overstaying

Find your date on day one

Unless you already hold a long-term pass, ICA emails you an e-Pass after you clear immigration, to the address on your SG Arrival Card. It shows the last day of stay allowed. If the email goes missing, ICA's e-Pass Enquiry Portal on the SG Arrival Card e-Service pulls it up from your D/E number or passport details. Diary that date the day you land, before jet lag and onboarding swallow the week, and send it to HR in writing.

If issuance beats the date, you are fine

MOM's FAQ is plain about this: once the EP is issued in EP eService, you don't need to extend the visit pass, because issuance validates your stay. So the cleanest fix for slow labs is chasing the clinic hard enough that HR can issue before your last day.

If it won't, HR asks MOM for more time

MOM's apply page lets employers extend the STVP of a candidate who's waiting on medical results. The request goes through MOM's own online form, "Request for more time to get the pass issued/renewed or send pass holder home", and the company (or its employment agent) files it. The form says the company representative can't be the pass holder, so you can't do this one yourself.

Timing is tight. Submit 7 to 14 days before the STVP expires. MOM's own example: if the STVP expires on 17 February, the request can only go in from 4 to 10 February. MOM warns that overstaying fines may be incurred for a late request, and the form spells out that it's the pass holder who may incur them. MOM says it emails the outcome within 7 days, which is another reason not to file on the last morning of the window.

What to hand HR so they can file without chasing you:

  1. The doctor's memo stating the medical results collection date. Get a named date on it, not a vague "in about a week".
  2. Your full name, FIN, passport number and passport expiry date, and the date you arrived. These are the personal details MOM's form asks for.
  3. Your visit pass end date from the e-Pass, so HR can count back to the window themselves.

This is MOM's issuance route, filed by your employer, not a tourist extension. ICA runs a separate online extension for visitors, which is what STVP extension covers. Don't let anyone assume one stands in for the other.

Stay in Singapore while you wait

Your IPA is a single-entry visa, and MOM needs you in Singapore on the day HR issues the pass. You used the IPA's single entry when you landed, so a weekend in JB or a quick trip home mid-wait leaves you trying to get back in without it. Stay put until the Notification Letter arrives. That letter is what lets you travel in and out while the card is printed: work pass issuance, EPSC and the card.

Quick decision table

If…Then…
Your IPA says a medical isn't requiredFollow HR's declaration path, and don't invent a WP medical
Your IPA asks for a medical declarationComplete it as the form directs and get it back to HR quickly
Your IPA asks for a full examBook a Singapore clinic in your first days, not in month five of a six-month IPA
Results land before your visit pass endsHR issues the pass, and no visit pass extension is needed
Results will land after your visit pass endsHR files MOM's request 7 to 14 days before expiry, with the doctor's memo
HR sends you Form A11 or another WP medical PDFAsk for the EP-specific form, or confirmation from the IPA attachment
You're appealing, or the IPA never issuedThe medical is irrelevant until there's an approval: EP rejection / appeal

Next: finish issuance, EPSC, and the card, then work through the SIM, Singpass, and bank map.

Questions, answered

Do all Employment Pass candidates need a MOM medical exam?
No. Your IPA letter states whether a medical examination is required after arrival. MOH told Parliament in March 2026 that selected EP applicants submit a medical declaration that they are free of TB instead of having a chest X-ray. Follow your own IPA, not a colleague’s.
Is the MOM medical_form.pdf on Google the right form for EP?
Almost certainly not. That file is Form A11, MOM’s full medical examination form for issuing a migrant worker’s Work Permit, and its results go into a separate MOM portal. Use only the form or declaration attached to your IPA or supplied by HR for the EP eService upload.
Can I do the EP medical overseas before I fly?
Not the chest X-ray. MOM’s apply page puts the medical after your arrival in Singapore, and MOH describes the X-ray screening as done on arrival. A recent check-up at home doesn’t replace the form your IPA names.
What if my STVP expires while I wait for medical results?
Your employer can ask MOM to extend the STVP, submitting the request 7 to 14 days before it expires with a doctor’s memo stating the results collection date. MOM’s example: an STVP expiring on 17 February means a request window of 4 to 10 February. If HR can issue the pass before the STVP ends, no extension is needed at all.
What do I need to give HR so they can file the extension?
The doctor’s memo with a named results collection date, plus your full name, FIN, passport number and expiry date, and the date you arrived, which are the personal details MOM’s request form asks for. Add your visit pass end date from the ICA e-Pass so HR can count back to the window.
Will ICA accept that the clinic was slow if I end up overstaying?
Don’t count on it. MOM’s request form says it must go in at least 7 days before the STVP expires, otherwise the pass holder may incur overstaying fines. The clinic’s timetable doesn’t move your visit pass date, so the request has to go in on time.
Can I start work while the EP medical is still pending?
No. An IPA isn’t permission to work. You can start once HR has issued the pass and the Notification Letter has arrived, and issuance waits on whatever medical form or declaration your IPA asks for.
Does a recent company physical replace the EP medical?
No. If your IPA asks for a medical, HR uploads the completed medical examination form or medical declaration form when issuing the pass in EP eService. A pre-employment physical done for insurance, or last year’s corporate check-up, isn’t that form.
